List all pieces to be performed. Current maximum is 2 pieces. PLEASE SEE THIS LINK TO ENSURE THE DURATION OF EACH PERFORMANCE IS WITHIN THE GRADE TIME LIMIT.
If the Composer is not present use OTHER and enter the Composer’s name manually.

Accompanists (Leave BLANK unless you are an instrumentalists, a singer or a pianist using accompanists for concertos).

If you are requesting a particular concert time, please provide a VALID reason. We will attempt to accommodate any special requests but note that this is no guaranteed. Notes are not mandatory and can be left blank.
